Bridal Agreement

The purpose of this agreement is to confirm the services to be rendered for the wedding party along with the name(s) of those responsible for payment of services provided by Epiphany Salon. Once this agreement has been filled out and signed the services will be scheduled. Epiphany Salon requires seven calendar days notice of any cancellation(s) of appointments listed on this contract. If, for any reason any member of the party does not show up for a scheduled appointment(s), and does not give seven calendar days notice of cancellation, the party responsible for this contract will be held responsible for the payment in full of any such services.

This agreement must be completed before scheduling of services. After you have submitted your information, please call the salon to make sure the date is still available and we have received your contract.

Helpful Hints and Suggestions

Bring your headpiece, veil, flowers, hair ornaments, or any other special touches you want the stylist to add to your special look. If you are having your make-up done, you need to bring water-proof mascara. (Aveda does not have water-proof mascara).

Wear something with a button down front or with a large neck so you will not mess up your hair when you change into your formal attire.

Please remember to schedule your practice up-do.
